*IMMUNIZATION RECORDS FOR STUDENTS ENTERING THE 7TH GRADE MUST BE SUBMITTED BEFORE 8/10/18 TO AVOID EXCLUSION FROM SCHOOL FOR NONCOMPLIANCE
*ALL 7TH GRADERS MUST RECEIVE A COMPLETE SCHOOL PHYSICAL EXAM-THIS WILL ALSO BE THEIR SPORTS PHYSICAL
*PHYSICAL EXAMS FOR 7TH GRADERS SHOULD BE OBTAINED AFTER 3/1/18 AND COMPLETED BY AUGUST 2018-IF THEY ARE PARTICIPATING IN A SPORT-THIS MUST BE SUBMITTED PRIOR TO PARTICIPATION
*ALL 8TH GRADERS MUST HAVE A SPORTS PHYSICAL DATED AFTER 5/1/18 AND SUBMITTED BEFORE THEY CAN PARTICIPATE IN ANY SPORT
*ALL STUDENTS NEW TO NEBRASKA MUST RECEIVE A COMPLETE SCHOOL PHYSICAL. (Please contact your family physician or clinic if you are unsure of your child's status regarding immunizations.)
*IF YOUR CHILD DOES NOT HAVE A SCHOOL PHYSICAL, THEY WILL NOT BE ABLE TO COMPETE IN SCHOOL SPORTS ACTIVITIES
